The following must be on file in the Athletic Office for students to be eligible to participate in athletic activities:

Step 1 MSHSL form: All pages MUST be completed by student/parent/guardian for the year if they are participating in one of the following....Soccer, Football, Volleyball, Cross Country, Basketball, Dance Team, Wrestling, One Act Play, Track and Field, Softball and Baseball

• Spanish MSHSL Eligibility Form

Step 2 Sports Physicals: Physicals are required every three years. Most athletes will get their physicals their 7th and 10th grade years.










 

• English Sports Qualifying Physical Form
• Spanish Sports Qualifying Physical Form

Step 3 Participation Fees: $100 per activity, with a $400 family max per year.

• Fees can be paid online through RevTrak HERE, cash or check.
   

Step 4 Eligibility Ticket: Once the steps above are competed, the student will be given an eligibility ticket to turn into their coach on the first day of practice. Please note, students will not be able to begin practice until the steps above are completed and they have the eligibility ticket.

TRANSPORTATION FORM:  Students are expect to ride home on the bus after sport events or activities.  However, we understand there are times they are needing to ride home with someone else.  Please complete the transportation form, with names of those you are giving us permission to allow your student to ride home with.
